First 12 minutes of Malpraxis gameplay show medical horror aboard a doomed ship

Sunscorched Studios has released a video showing the first 12 minutes of gameplay from Malpraxis, a game blending medical simulation, psychological horror, and investigative drama. It's set to launch on PC on February 27, 2026.

The footage covers the S.P.I.D.E.R. unit startup sequence, a pre-shift briefing, a routine calibration exercise that quickly spirals into chaos, and the opening of a crew medical checkup.

Players take control of the S.P.I.D.E.R. unit – a neural-integrated system developed by REINERSTAHL Industries to assist Chief Medical Officer Samuel Edwards aboard the TRH Rusanov.

Gameplay revolves around three core disciplines: triage, diagnosis, and surgery. Every crew member who enters the operating room is the player's responsibility. Edwards provides support, but the key decisions fall to the S.P.I.D.E.R. unit, and patient survival depends on how quickly its algorithms are calibrated.

Every decision in Malpraxis carries weight. Edwards is bound by the Hippocratic oath, and the game's moral stakes extend well beyond the unit's core directives – each choice during a procedure affects the doctor and his team. Outcomes may not align with what the system predicted, and patients live or die based on what you do.

There are no second chances. Lose a patient and they're gone – all that's left is to adapt and press on. The events of Malpraxis are framed as a reconstruction of an incident built around precision, tension, and trauma response, one that ultimately leads into the events of Negative Atmosphere, another title currently in development.
